How Out-of-Network Benefits Work

Many PPO insurance plans include out-of-network (OON) mental health benefits. If you have these benefits, your insurance can reimburse you for a significant share of your session fee, often between 50% and 80%. This can make your actual cost similar to, or even less than, high in-network copays, while still giving you access to specialized, integrative care.

First: Check Your Benefits

Before starting therapy, call your insurance to understand your coverage. Ask these questions:

  • What are my out-of-network benefits for mental health services?
  • What percentage do you cover for CPT code 90834 (individual psychotherapy) and other common CPT codes? (see below)
  • What's my out-of-network deductible, and how much have I met this year?
  • Is there a session limit for mental health services?
  • Common CPT codes: 90791 (Intake), 90837 (60 min), 90834 (45 min), 90832 (30 min)

Then: Getting Reimbursed (Step-by-Step)

  1. You pay for your session: All payments are handled securely through our client portal (SimplePractice). You'll keep a credit, debit, or HSA/FSA card on file, which will be charged after each session.
  2. We provide everything you need: After each session, we give you a detailed superbill with all the information your insurance requires.
  3. You submit for reimbursement: Upload the superbill to your insurance website or mail it in. Most clients get reimbursed within 2-4 weeks.
  4. Insurance sends you money back: You receive a check or direct deposit for your covered amount.

Questions You May Have

Call the member services number on your insurance card and ask the specific questions listed above, including whether pre-authorization is required. This conversation will give you the clearest picture of your benefits.

Online submissions are usually processed within 2-4 weeks, while mailed claims can take longer. Each plan has different timeframes, so your insurance can give you more specific expectations.

Most insurers let you upload your superbill through their member portal and fill out a simple form. If you prefer mailing, you'll need to include your insurer's claim form. We'll provide detailed superbills with everything you need.

Check your Explanation of Benefits first to see the reason for denial. Often it's a simple issue like a code or date that can be corrected and resubmitted.

Your therapy payments count toward your out-of-network deductible. Once you meet it, your plan begins reimbursing its percentage. Even before that, you're building toward future reimbursement.

We keep a credit card securely on file through our patient portal and charge it after each session. You'll complete a one-time authorization form during intake to set this up.

You can still use HSA/FSA accounts. We're also happy to discuss sliding scale options during your consultation to make therapy accessible.
